Affidavit In Lieu of a Survey
The owner of the real estate certifies the improvements (house, garage, outbuilding, etc.) on the property are within the boundary lines and set back lines and that there are no encroachments (fences, walkways, driveways, eaves, drains, etc.) by improvements on adjoining property. -

Affidavit of No New Improvements
An instrument executed by the owner of record, to inducement the title company to issue extended coverage on the Owner and/or Mortgagee Policies. -

Certification and Authorization
Authorization for the title company to obtain non-public information, including but not limited to, mortgage payoffs, liens and/or judgment payoffs, and any and all other information to clear title for the transaction. -

Personal Indentification Information Affidavit
Affidavit to assist the title company to determine whether or not certain matters of record will affect the real estate. -

Prescribed by Department of Local Government Finance Pursuant to IC 6-1.1-5.5 -

Strict Joint Order Escrow Instructions
Direction from the parties involved in a transaction to the title company who may be holding money or other pertinent documents. -
